<p>Recipients are to submit a REPORT at the end of the project or funding period. Failure to submit this report will disqualify recipients for future grants. Recipients are also requested to send PICTURES of events when they happen. These may be used in BGCF publicity, on the website, and in annual reports. Pictures should be sent electronically if possible to bgcf@bgohcf.org.</p>

<p><em><span style="text-decoration: underline;">Total Project Cost</span></em>: A line-item financial report delineating the actual income received and expenses incurred during the grant period. This report should be compared to the original budget submitted with the proposal and include an explanation regarding any significant variances between the proposed budget and the actual income/expenses. Please delineate the use of grant funds as related to project expenses, and provide a description of all other income sources used to support the project (if applicable).</p>

<p><em><span style="text-decoration: underline;">Expenditure of Grant Funds</span></em>: Please indicate the total amount of grant funds expended as of termination date of the grant period. Please remember that unexpended funds must be returned to the Bowling Green Community Foundation.</p>

<p><span style="text-decoration: underline;"><em>Future Funding Plans</em></span>: List all anticipated sources of income for the future, including approved or pending funding requests and their amounts.</p>

<p><span style="color: #000000;"><strong>IV. PUBLICITY</strong></span></p>
<p>A. Grantee organizations are required to make public announcements in respect to the grant, knowing such notices might stimulate additional support for new projects. Additionally, grantee authorizes BGCF to publicize the issuance of the grant and the purpose for which it was awarded.</p>

<p>B. Copies of press releases, photographs or any other published materials (regarding either the grant or the program funded), should be sent to the Foundation. We also request that you provide the Foundation with a list of where the press releases were sent.</p>

            <description><![CDATA[<p>The year 2012 grant awards supported the following projects and activities from the Bowling Green Community Foundation Unrestricted Fund:</p>
<ul>
<li>American Red Cross - To support the Ready U Plus Disaster Preparedness Program.</li>
<li>Assistance Dogs of America - To help train a therapy dog to assist victims of human trafficking at the Daughter Project.</li>
<li>BGSU Athletic Department - To provide tickets and T-shirts for 200 youth to attend the NCAA Women's Basketball Tournament games.</li>
<li>Bowling Green Pee Wee League, Inc. - Family needy fees, drainage upgrade, and additional fencing.</li>
<li>Historical Construction Equipment Association - To process and catalog company records and transfer motion pictures to DVD.</li>
<li>Montessori School of Bowling Green - To assist in the development of an educational outdoor trail.</li>
<li>Northwest Ohio Center for Excellence in STEM Education - To help fund STEM in the Park.</li>
<li>Wood County District Public Library - To purchase 70 sets of books and 70 magnetic alphabet sets for Every Child to Read program.</li>
<li>Wood County Humane Society - To help implement the TNR program in the Bowling Green area.</li>
</ul>
<p style="text-align: left;">The year 2011 grant awards support the following projects and activities from the Bowling Green Community Foundation Unrestricted Fund, with additional funds from the Green Family Fund, Marsh Family Fund, Pallister Fund and Town/Gown Fund:</p>

<li>Behavioral Connections of Wood County - To support the Wood County Suicide Prevention Coalition for a suicide prevention conference.</li>
<li>Black Swamp Arts Festival - Funds from Cormier Fund to support the expansion of two new activities for young children in the Youth Art Area.</li>
<li>BG Pee Wee League -To assist in paying fees for children from families who cannot afford the registration fee.</li>

<li>Bowling Green Parks and Recreation - Funds from BGCF and the Marsh Family Fund to assist in the development of a skate park for youth at Bowling Green City Park.</li>
<li>The Cocoon Shelter - To support an economic empowerment project to help victims of domestic violence develop financial independence.</li>
<li>The Connection Center, a program of the Behavioral Connections of Wood County - To provide funding to purchase computers to assist individuals with mental illness obtain updated educational/job skill resources.</li>
<li>Dear Santa Society - To assist in providing clothing and toys for families in need as identified by Bowling Green school personnel and the Salvation Army.</li>
<li>Diabetes Youth Services - To support programs including camp for youth with diabetes from the Bowling Green area.</li>
<li>Downtown Bowling Green - To support the continuance of the newly-organized Farmer's Market in downtown Bowling Green.</li>
<li>Friends of Wood County CASA - To train and support volunteers to become court appointed special advocates for neglected and abused children from the Bowling Green area./span&gt;</li>
<li>Children's Resource Center - To purchase materials for screening young children with sensory processing dysfunction.</li>
<li>Wood County District Public Library - To support the library's summer reading program.</li>
<li>Wood County District Public Library - Funding from BGCF and the Pallister Fund for the "Page Turners!" initiative, a program that allows community leaders the opportunity to share books that made a difference in their lives through the use of the library's Web site and Facebook.</li>
